The Carle Home Care team provides services ordered by a doctor, requiring intermittent skilled nursing, and or physical or occupational therapy for homebound patients. They ensure a smooth transition from a clinic, hospital or extended care facility, and train patients to care for themselves and their caregivers to manage pain. The Carle Hospice team of nurses, social workers, nursing assistants, chaplains and trained volunteers take an individualized, holistic approach to an end-of-life care philosophy. Our focus is to help patients live life as fully and comfortably as possible, promoting quality of life.
