OSF HealthCare Heart of Mary Medical Center Community Resource Center provides referral services to our patients and helps to connect them to an array of social services available in the community. We take a holistic approach to medical problems and aim to address unmet social needs of patients in the emergency department or directly at the Community Resource Center (CRC).
